My name is Kathy Proctor, and I'm a Registered Nurse, Board‑Certified Case Manager, with a Master's in Applied Clinical Nutrition. I created the Postpartum Clinical Bridge Program because I know, both professionally and personally, how critical the postpartum period truly is.
At 36 weeks pregnant, I was diagnosed with preeclampsia. I also had a history of a congenital heart condition, and I knew that my risk profile required more than routine postpartum care. I advocated for myself, requested referrals to a cardiologist, and retrieved my childhood cardiology records to ensure my care team had the full picture. That advocacy helped protect both my daughter and me, who is now 20.
But not every mother knows what to ask for. Not every mother has a nurse's background. And not every mother has someone watching closely during the most dangerous weeks after birth.
That's why I built this service.
What I Do
I support mothers recovering from:
Preeclampsia
Gestational diabetes
Hypertension
Complicated pregnancies or births
The postpartum period is not "the end"; it's a clinical transition that requires monitoring, education, and early intervention. Many complications peak within the first 10 days after discharge, yet most mothers don't see their provider again for six weeks.
My program bridges that gap through:
Daily remote blood pressure and blood glucose monitoring
Clinical triage and symptom checks
Postpartum assessments for mothers and partners
Nutritional and lifestyle guidance to support healing
Clear red‑flag escalation protocols
Continuous support for up to 12 months postpartum
Why It Matters
Because too many mothers are sent home without the support they need. Because preventable complications still happen every day. Because maternal health shouldn't depend on luck, timing, or who happens to be available.
I created this program to be the clinical eyes and ears in the home; the support I wish every mother had.
